Daycare & Boarding Membership Form
- My dog will attend an initial trial day, to help my dog settle in the pack and to assess my dogs suitability for K9 Heaven’s social environment.
- I understand that K9 Heaven operates in a ‘pack’ environment and therefore accept all of the benefits and risks associated with this and agree that K9 Heaven is not responsible for any injury or illness, or the loss or damage of any belongings, food or medication left in our care during my dog’s attendance at K9 Heaven.
- I understand that I am liable for any veterinary/medical care expenses and damages that result from injuries caused by my dog.
- I give permission for K9 Heaven to seek medical attention for my dog if any medical issues develop while my dog is in its care and I agree to pay all costs associated with treatment.
- I understand that K9 Heaven reserves the right to refuse access to any dog that displays unstable, unsociable or aggressive behaviours, and if I am not willing to engage with our dog behaviourist to rectify these types of behaviours first.
- I agree to not hold K9 Heaven, its employees and representatives, liable for any injury/loss/death/sickness of my dog.
- I understand that under no circumstances will K9 Heaven be liable for subsequent damages or damages which are beyond the replacement value of my dog.
- Opening hours are from 7:00am to 6:00pm, 7 days a week. I agree that if I’m unable to collect my dog by 6:00pm for daycare, my dog will be boarded overnight at my expense. This will be charged at the current daycare rate plus the current overnight boarding rate.
- All dogs boarding overnight are required to be collected the following day by 12:00pm, at no extra charge, however if my dog is collected after 12:00pm a full day care charge will apply.
- All fees must be paid at time of drop off or pick up of my dog when boarding. For daycares, fees must be paid in advance or at the end of each week. No accounts unless by prior approval.
- I understand that if I fail to complete payment for boarding within 3 days of collecting my dog I will be liable for all collection costs or 3rd party costs associated with the collection of any unpaid invoices. I understand a $7 post payment fee will be applied to accounts not paid for within 3 days of pick up from K9 Heaven. A $7 post payment fee will be added on the total bill every 7 days thereafter until 1 month amounting to $28.00 due of administration fees. Please see website for further details on unpaid invoices.
- I agree to inform K9 Heaven of any of my dog’s behaviours initially and as they develop that may impact K9 Heaven’s decision on which area to keep my dog and where they may be exercised. This includes but is not limited to behaviours such as jumping, escaping, bolting, aggressive behaviours towards people, dogs or other animals or a menacing classification being placed on my dog.
- I understand and agree that should my dog “escape” K9 Heaven’s property due to but not limited to: bolting on a pack run, jumping fences, breaking or digging out of an area, breaking out of enclosures by any method, K9 Heaven will not be held liable for any costs associated or any damages caused to property, other animals or persons. I agree that my dog is not of the escape type and have notified K9 Heaven of any issues regarding, I agree with K9 Heaven’s terms regarding my dog escaping. K9 Heaven does not and will not admit ‘escape’ type dogs and I have been honest and forthcoming regarding my dogs behaviour’s. I have read this and understand K9 Heaven reserves the right to seek compensation as appropriate.
- Unless stated on my membership form, I agree to K9 Heaven “pack running” my dog on the 12 acre farm, and accept all risks associated with this.
- I understand that K9 Heaven is bound to the Dog Control Act & the Animal Welfare Act, and any such actions, situations, incidents or events occurring with my dog may require K9 Heaven’s attention to these Acts or anything associated and forthcoming.
- Seeking and having service given by K9 Heaven in written form or verbal, is considered to be both legally bound as ‘service provided’, at no time does a written or verbal agreement deter from K9 Heaven terms and conditions as written here (official website). A dog placed into K9 Heaven’s care (daycare, boarding, grooming, international pet transport, foster) is considered ‘service provided’ through written or verbal agreement. I understand K9 Heaven reserves the right to amend terms and conditions at any time. An up to date copy will be placed on K9 Heavens website and paper copies will be available in the office.
- I understand that all daycare and boarding packages are valid for 12 months from the date of purchase and are non-refundable. Boarding packages cannot be used during peak periods (including but not limited to eater, christmas, new years and long weekends), and standard boarding rates will apply during these times. It is my responsibility to confirm availability and ensure package usage falls outside of peak dates.
Grooming Membership Form
- Please tell us if your dog has ever had any health or behavioural problems, so we can ensure the correct procedures are used to protect the health and safety of ourselves and other dogs.
- All dogs should be vaccinated. While K9 Heaven endeavours to ensure the well being of all dogs in our Grooming & Pampering service, we cannot guarantee against every situation, and so will not be liable for consequences of any unforeseen circumstances.
- Please inform us if your dog has displayed previously any anxieties or aggression while being groomed. We reserve the right to refuse to or to stop grooming if we feel your dog has become too stressed or aggressive.
- During the grooming process all care is taken to ensure the health of your pet is considered. K9 Heaven is not liable for any pre-existing conditions and problems found during grooming.
- In the event of a matted coat needing to be clipped short and close to the dogs skin, the owner should be aware that irritation may occur from the shaving process. Due to mats sometimes being tight to the skin, the dog’s skin may get caught because of the close proximity of working to the skin. We will always attempt to maintain as much hair as possible when matting is evident however, we maintain the right to shave matting where required to prevent discomfort to your dog.
- The owner agrees to pay all costs for the grooming of the dog including de-matting. An estimated price may be given at the time of booking or at drop off of the dog however, K9 Heaven reserves the right to change the price to include de-matting time if this is required.
- The dogs health & safety at the time of groom will be considered as first priority at all times.
- Payment is to be made at the time of pick up or drop off.
- Refunds will not be given if you are not satisfied with the groom provided. However, we will endeavor to schedule another appointment with your dog within 7 days to tidy any areas you may not be happy with.
- I understand that if I cancel my grooming appointment with less than 24 hours’ notice or fail to attend, I am liable for the full cost of the scheduled service.
